Please complete the information below for EACH and EVERY week your child will participate in Summer 2 Serve. This will help us make sure we are well prepared with supplies and volunteers.

Jana Whittle will come speak to the kids about Operation Christmas Child and the shoebox ministry. 

We will then make gift bags for the VBS kids in Peru!

Please fill out the following Permission Slip for your child to participate in the weekly activities for Summer 2 Serve. This includes permission for your child to be transported in a vehicle operated by an approved and licensed adult chaperone while attending Summer 2 Serve activites. This also includes medical treatment release.

This only needs to be filled out ONE time for the duration of 2025 for each child.
